From your continued descriptions, it still sounds like normal perspective distortion. And I'll say again that it would help solve the whole thing by letting us see the photo, unless you don't have any way to scan it?

Setup the camera. Check the ground glass. You should see the exact same result on the ground glass. No need to shoot film. If you don't see the problem with no film in the camera then I guess the issue might be related to how the film holder is inserted.

I agree with the comments that it just sounds like the back is tilted. But you can't change the back position. Did you need more rise then the camera allowed? This is why I wish my press camera had rear tilt. Just adding rear tilt would save having to haul the much heavier monorail around.
